To use this tool, the HIC must be onboarded (either the physician or the organization). This includes completing the intake form and obtaining a OneID. Please note the waiting time to complete the process can take up to six weeks.

Health Care Connect (HCC) is a provincial program that connects unattached patients to a family doctor or nurse practitioner in their community when one  becomes available.

The HCC program now accepts attached patients who are seeking a primary care provider closer to home. Hastings Prince Edward residents who currently have a family physician or nurse practitioner outside of the region can register with HCC.

By posting the following posters in high traffic areas, such as waiting rooms and community bulletin boards, HPE OHT Partners will help achieve our goal of attaching our community members to primary care.
