    Offered through Lennox & Addington County General Hospital. Offering exceptionally short wait times for MRI exam completion — from booking to final report:
    • < 10 days for P3 (urgent) exams
    • ~28 days for P4 (routine) exams

Lower Limb Preservation Demonstration (LLPD) ProjectFoot Care
This project includes a cross-sector collaboration of partner organizations that focus on detecting early identifications of patients/clients at risk through standardized assessment and providing timely access to vascular surgery for high-risk patients.

Ocean eReferral Network

Referring patients to specialists has never been easier with Ocean eReferral Network. This simple and secure channel easily transfers patient information from one point-of-care client management to another.

Features include auto-populating patient demographics and (most) required medical information right from your EMR–saving you time. Want to know the status of the referral? This platform keeps you updated through your EMR and with email notifications to you and your patients. ConnectingOntario Clinical Viewer

Through Ontario’s digital health advancements, family physicians and nurse practitioners can view a more complete picture of their patients’ health journeys—no matter where their care was received across the provincial system.

This real-time access to clinical information supports quicker, better-informed decision-making, streamlines workflows, and helps reduce unnecessary tests.

To use this tool, the HIC must be onboarded (either the physician or the organization). This includes completing the intake form and obtaining a OneID. Please note the waiting time to complete the process can take up to six weeks.

Communications & Promotional Material

Health Care Connect

The following material is for all HPE OHT Partners to promote Health Care Connect.

Health Care Connect (HCC) is a provincial program that connects unattached patients to a family doctor or nurse practitioner in their community when one  becomes available.

The HCC program now accepts attached patients who are seeking a primary care provider closer to home. Hastings Prince Edward residents who currently have a family physician or nurse practitioner outside of the region can register with HCC.

By posting the following posters in high traffic areas, such as waiting rooms and community bulletin boards, HPE OHT Partners will help achieve our goal of attaching our community members to primary care. We also make social media posts and written copy for partners. If you have any questions, please contact that Backbone Team HERE.

Land Acknowledgement

HPE OHT delivers supports and services on lands that are the traditional territory of many nations, including the Huron-Wendat, Anishinaabe, Algonquin and the Haudenosaunee peoples.
