Primary Care Network
The HPE OHT Primary Care Network is a collaborative of family physicians, nurse practitioners (NP), administrators, allied health providers (AHP) and patient/caregiver/community representatives who are committed to improving our local primary care system. This includes access, attachment, quality, and coordination of services across the Hastings Prince Edward communities.
It is through the PCNs input, feedback and recommendations that system-level planning begins from the ground up.
Our primary care partners are welcome to join upcoming PCN meetings. Please email info@hpeoht.ca to begin receiving the meeting invitations and agenda.
