Cultivating Connections: Celebrating 5 Years of Growing Care Together - Oct. 29/26

On October 29, the HPE OHT partners came together to celebrate five years of collaboration and the collective impact of their work across Hastings Prince Edward’s health and social service systems.

The event opened with a poster abstract display featuring collaborative initiatives from across the region. These projects highlighted the partnerships, ideas and pathways being developed to improve experiences for patients, clients, families and the people who provide care and services.

Guests also heard directly from leaders involved in several collaborations, who shared their successes, challenges, and lessons learned along the way. Their stories reflected the complexity of bringing organizations and people together to create more connected experiences for patients and clients across Hastings Prince Edward.

Systems change takes time, trust and a willingness to work through the messy parts. As we celebrate how far we’ve come, we remain committed to learning, strengthening relationships and building on this work together.

Poster Abstracts

Thank you to everyone who submitted poster abstracts and shared the collaborative work taking place across Hastings Prince Edward.

Cultivating Connections – From Seeds to Systems

Local graphic recorder, Shannon Loomer, joined us to translate the conversations, connections and ideas shared throughout the event into a visual representation of the HPE OHT’s collective work.

Working together through the messy

Building a Picture of Population Health in Hastings Prince Edward - Oct. 7/26
This presentation highlights the Hastings Prince Edward Ontario Health Team’s (HPE OHT) ongoing work to use population-level data to better understand the health and wellness needs of our region. Recognizing that population health extends beyond those currently accessing health and social services, the HPE OHT is working to make data more accessible, visible and useful to inform planning, decision-making and regional initiatives. Sally Fan, a recent Master of Public Health graduate from Western University, shares insights from her summer practicum project, exploring population health data sources and opportunities to strengthen our understanding of the people and communities across Hastings and Prince Edward.

Land Acknowledgement

HPE OHT delivers supports and services on lands that are the traditional territory of many nations, including the Huron-Wendat, Anishinaabe, Algonquin and the Haudenosaunee peoples. It is also directly adjacent to the Kenhte:ke – the Tyendinaga Nation. These territories are governed by the Dish with One Spoon wampum agreement. We share a responsibility to acknowledge and respect the culture and history of Indigenous peoples, who have inhabited and nurtured these lands for generations. We commit to centering our work of overall health and wellbeing for all residents, with particular consideration for Indigenous peoples and all equity-deserving populations in Hastings and Prince Edward Counties.
