Priorities

The 4 priority areas listed below are the areas of focus for the Hastings Prince Edward Ontario Health Team that will enable us to achieve better health and wellness, within, and across our communities. 

Integration of Mental Health & Addictions with Primary Care

Strengthen integration of mental health and addiction (MHA) services with primary care providers by
reviewing existing MHA services and creating an effective referral and intra-professional
communication system.

Strengthening our Primary Care Network

Develop and strengthen the local primary care network.

Fall & Winter Preparedness Response & Recovery

Create a collaborative approach between all relevant member organizations, to ensure that every
person in the OHT area who wants a COVID-19 vaccination has received one. This will include identifying
marginalized people, assisting them with education about COVID-19 and the value of vaccinations,
facilitating transportation to a vaccination centre, or delivering the vaccination to them.

Furthermore, this priority area also includes OHT-wide fall and winter preparedness efforts, including coordinated efforts to respond to a challenging cold and influenza season. 

 

Home Care Modernization & Redesign

Develop a model(s) for homecare integration within the OHT to enhance timely access to services,
increase equitable access, and ensure effective direct communications with primary care providers.
